
About RISE Care
Jocelyn Rempel and Gail Crockford are registered nurses, educators, and innovators with a shared passion for transforming how we care for older adults. Together, they bring over 40 years of combined experience across acute, community, and academic settings, as well as master's degrees in nursing and health-care policy. Both are certified by the Canadian Nurses Association in Gerontological Nursing and have dedicated their careers to advancing evidence-informed, person-centred care for aging populations.
Jocelyn is the founder and chair of the Centre for Health and Innovation in Aging, where she leads initiatives that bridge research and practice. Gail has led clinical programs and contributed to policy development, while also shaping future generations of nurses through her teaching and mentorship. As active members of the Centre, both founders are committed to putting research into action—creating practical, scalable solutions that address the real-world challenges older adults and caregivers face every day.
Rise Care was born from this vision. Jocelyn and Gail founded the company to close the gap between knowledge and practice in home and community care. Their mission is clear: to improve the quality of life for older adults by equipping caregivers, families, and organizations with tools, training, and systems that work.
Outside of their professional lives, Jocelyn and Gail are also busy moms and wives with children ranging from elementary school to university. They share a love for family, road cycling/activities, travel, and being part of a community. Their friendship, shared values, and drive to make a difference are at the heart of Rise Care.
